Medical necessity

Request a Medical Necessity Review

Have a medical device, supply, or health-related product you believe you need? ThriftMeds can help organize the documentation and provider review process so you can better understand what information may be required before pursuing coverage or fulfillment.

What is medical necessity?

Certain Medicare-covered medical equipment, supplies, and services may require documentation showing that an item is reasonable and necessary for your condition. Medical necessity is generally supported by appropriate clinical documentation and provider involvement—not by retail listings or a signature alone.

What can be requested

Coverage depends on the applicable Medicare or Medicare Advantage benefit, medical necessity, eligibility, documentation, authorization, supplier requirements, and plan rules—not every item listed below is covered for every beneficiary.

    • Walkers
    • Canes
    • Wheelchairs

Found an item online?

A product being available for purchase from a retailer or marketplace does not automatically mean Medicare or a Medicare Advantage plan will cover it. If you identify an item from a retailer such as Amazon or another marketplace, ThriftMeds may help organize a request for provider review where appropriate.

Retail availability
Medicare coverage
Medical necessity
Plan authorization

ThriftMeds does not represent that any retailer is an authorized Medicare DME supplier for your plan unless explicitly verified through your plan materials.

How it works

ThriftMeds does not replace your Medicare Advantage plan's coverage or authorization process.

  1. 1

    Identify the item

You identify the medical device, equipment, or supply you are seeking—whether from a plan formulary, a supplier catalog, or a retailer listing.

  2. 2

    Start a request

You provide information about the item, your coverage, and how to reach you. ThriftMeds organizes the request for provider review.

  3. 3

    Invite the provider

You identify the healthcare provider who can evaluate whether the item is clinically appropriate for your condition.

  4. 4

    Provider review

The provider reviews the request and determines whether the item is clinically appropriate—not whether Medicare will pay for it.

  5. 5

    Documentation

The provider supplies documentation or orders required for the applicable coverage or supplier process, when appropriate.

  6. 6

    Electronic signature

Where a secure e-signature workflow is enabled, the provider may complete documentation electronically. Signature records participation—it does not establish coverage.

  7. 7

    Coverage review

Documentation may be submitted through the applicable Medicare, Medicare Advantage, or authorized supplier process for a coverage determination.

  8. 8

    Fulfillment

If approved through the applicable process, the item is obtained through an authorized supplier or retailer as plan rules require.

Electronic signature workflow

Provider documentation

ThriftMeds may use a secure electronic-signature workflow to allow healthcare providers to review and complete documentation electronically when that capability is enabled for your request.

Where applicable, the workflow may generate:

  • Provider signature and timestamp
  • Transaction identifier and completion record
  • Document history and audit information

The electronic signature documents the provider's participation. It does not independently determine Medicare coverage. DocuSign or similar integration is planned where not yet connected—no envelope is created until that workflow is live for your request.

Who decides whether it is covered?

The final coverage determination belongs to the applicable Medicare program, Medicare Advantage organization, or authorized coverage and supplier process—not ThriftMeds.

  • Medicare rules
  • Medicare Advantage plan rules
  • The applicable benefit category
  • Medical necessity standards
  • Provider documentation
  • Prior authorization requirements
  • Supplier and network requirements
  • Product classification
  • Applicable coverage policies

Before you buy

Prior authorization

Some items may require prior authorization before Medicare or your plan will cover them. A provider's recommendation does not necessarily mean the item can immediately be purchased and reimbursed.

If prior authorization is required, the appropriate process must be followed before fulfillment when applicable.

What the provider may need

Exact requirements depend on the item and coverage pathway.

  • Beneficiary information
  • Medical condition and relevant clinical history
  • Functional limitations
  • Description of the requested item
  • Quantity and duration of need
  • Provider name, credentials, and contact information
  • Written order where required by the applicable process
  • Provider signature
  • Supporting medical-record documentation
  • Additional documentation required by the applicable plan

Request status

A typical pathway—your request may not reach every stage, and no stage guarantees approval.

  1. Request Created
  2. Provider Invited
  3. Provider Reviewing
  4. Documentation Completed
  5. Coverage Review
  6. Authorization
  7. Fulfillment

FAQ

No. A provider signature documents clinical participation in the review process. Medicare, Medicare Advantage plans, and authorized suppliers make separate coverage and authorization decisions.
